It is traditional, when saying Yizkor, to remember the souls of our loved ones by mentioning their Hebrew name, and their father's Hebrew name. If you have that information, please include it in the form below.

All names submitted will be read aloud by the chazan during the Yizkor service.

In order to be sure that your names are included, please make your submissions by Wednesday evening, September 16, at 5:00pm.
